Divergent implant
fixtures and an upper complete denture with teeth offset buccally & encroaching on the inter-occlusal space require planning
and collaboration between dentist and technician. With sound communications and case-planning, these anomalies were
overcome as seen in the following photos.

Maxillary
right quadrant: Denture teeth removed from CD base and repositioned to facilitate room for mandibular right implant prosthesis..This
solved the clearance issue, however, the divergent fixture problem had yet to be solved.

Path of insertion
established for 3i Gingihue posts with surveyor..an angled (15 degree) posterior abutment post was utilized to facilitate
this procedure.

Following adjustment
of the Gingihue posts, a duplicate cast was made and the framework waxed upon it & subsequently cast in gold..It
was verified to fit perfectly. A new occlusal registration was taken at this juncture and the case was re-mounted for
precise application of ceramic materials to the gold substructures.

Original mounting
reveals encroachment of posterior right denture teeth. The Wilson curve was obviously ignored when this denture
was made. #3-6 were no doubt set in haste since they opposed no contra-arch antagonists. When I removed them from the
base to re-set them, I found these acrylic teeth untouched with diatorics intact - as if transferred from the tooth
card and waxed directly onto the record base prior to processing with no consideration given to subsequent restorative
procedures for the opposing mandibular quadrant.

Finished Implant-bourne
casework provides this patient with an esthetically appealing, right posterior appliance. The movement of the maxillary
teeth on the denture base has augmented the overall stability of the occlusion, & effectively enhanced masticatory function. When the operatory and laboratory work together, a confident smile and the ability to ingest nourishment without pain, worry or embarrassment,
is the end result for the patient. Success with difficult casework is an extra- special reward for us.

3i Gingihue posts are refined and enhanced
with anti-rotational and retentive features. This is better accomplished in the lab with an abutment holder to protect the
precision machined surfaces and tactile elements.
